Registration Guidelines for Health Facilities
HOSPITALS
A Hospital is a healthcare institution appropriately staffed and equipped to investigate, diagnose and treat patients. It is a medical treatment facility capable of providing continuous services for acute and complex conditions including medical care, nursing care, outpatient, inpatient care and other relevant professional care.
Specialized medical and nursing staff, as well as other healthcare professionals, provide patient care in the hospital. A Hospital is expected to provide 24 hours service.
MINIMUM REQUIREMENTS
In addition to meeting the standards described below, all hospitals shall possess a corporate affairs commission (CAC) certificate. The Practitioner-In-Charge shall be fully registered medical practitioner with a minimum of 5 years’ experience. (S)he shall have a valid license from the Medical and Dental Council of Nigeria (MDCN) and shall be a full-time staff.
DENTAL LABORATORY
A Dental Laboratory is a laboratory where the construction of dental and maxilla facial prostheses takes place.
MINIMUM REQUIREMENTS
In addition to meeting the standards described below, all dental laboratory shall possess a Corporate Affairs Commission (CAC) certificate. The Practitioner-In-Charge shall be a fully registered with requisite experience. (S)he shall have a valid license from the Dental Technologist Registration Board of Nigeria.ORAL PATHOLOGY LABORATORY
An ORAL Pathology Laboratory is a specialized facility where dental/Oral and Maxillofacial pathologists and laboratory scientists receives, process and evaluate biopsy and surgical tissue specimen for the purpose of diagnoses of Oro-facial diseases, abnormalities of the teeth, gums and related oral structures.
MINIMUM REQUIREMENTS
In addition to meeting the standards described below, all dental laboratory shall possess a Corporate Affairs Commission (CAC) certificate. The Practitioner-In-Charge shall be a fully registered with requisite experience. (S)he shall have a valid license from the related Registration Board.DIAGNOSTIC CENTRE
A Diagnostic Centre is a centre for consultation investigation or diagnosis and follow up of medical disorders where ionizing radiation may also be used.
Diagnostics form a critical input for medical treatment, and the scope of services comprises either X-Rays and Ultrasound Scan (USS) or Pathological services or Medical Laboratory Services.
Pathological services include and not limited to histopathological, haematological, medical microbiology and chemical pathology.
MINIMUM REQUIREMENTS
In addition to meeting the standards described below, all diagnostic centres shall possess a Corporate Affairs Commission (CAC) certificate and Nigerian Nuclear Regulatory Authority (NNRA) authorization for operation (for diagnostic centers that uses ionizing radiation). The Practitioner-In-Charge shall be a fully registered Pathologist, Radiologist with appropriate experience. (S)he shall have a valid license from the appropriate regulatory body.EYE HOSPITAL
An Eye Hospital is a centre for investigation, diagnosis and treatment of all ocular disorders (medical, surgical and optical). Eye Hospitals can also engage in visual health promotion and prevention services. These establishments are expected to provide 24 hours’ service.
MINIMUM REQUIREMENTS
In addition to meeting the standards described below, all eye hospitals shall possess a Corporate Affairs Commission (CAC) certificate. The Practitioner-In-Charge shall be a fully registered ophthalmologist. (S)he shall have a valid license from the Medical and Dental Council of Nigeria (MDCN) and a certificate from the National Postgraduate Medical College of Nigeria or its equivalent that is registerable by the Medical and Dental Council of Nigeria (MDCN).MULTISPECIALTY HOSPITAL
A Multispecialty Hospital is a hospital that provides care for patients with ailments referring to specialized branches of medicine. Such a hospital shall be operated by a specialist in that branch of medicine. This category of hospitals offers TWO OR MORE specialized services provided by qualified professional staff. A Multispecialty Hospital is expected to provide 24 hours’ service.
MINIMUM REQUIREMENTS
In addition to meeting the standards described below, all multispecialty hospitals shall possess a Corporate Affairs Commission (CAC) certificate. The Practitioner-In-Charge shall be a fully registered medical practitioner who holds a Category A certificate of registration of additional qualification in that specialty with the Medical and Dental Council of Nigeria (MDCN). (S)he shall have a valid license from MDCN and shall be a full-time staff.PHYSIOTHERAPY CLINIC
A Multispecialty Hospital is a hospital that provides care for patients with ailments referring to specialized branches of medicine. Such a hospital shall be operated by a specialist in that branch of medicine. This category of hospitals offers TWO OR MORE specialized services provided by qualified professional staff. A Multispecialty Hospital is expected to provide 24 hours’ service.
MINIMUM REQUIREMENTS

The Practitioner-In-Charge shall be a fully registered medical practitioner who holds a Category A certificate of registration of additional qualification in that specialty with the Medical and Dental Council of Nigeria (MDCN). (S)he shall have a valid license from MDCN and shall be a full-time staff.SPECIALIST HOSPITAL

Such a hospital shall be operated by a specialist in that branch of medicine. This category of hospitals offers specialized services provided by qualified professional staff. A Specialist Hospital is expected to provide 24 hours’ service.

A Dental Clinic is a healthcare institution where services relating to the care of the teeth, oral cavity, and associated structures are provided on an outpatient basis.
MINIMUM REQUIREMENTS
In addition to meeting the standards described below, all dental facilities shall possess a Corporate Affairs Commission (CAC) certificate. The Practitioner-In-Charge shall be a fully registered dental surgeon with requisite experience. (S)he shall have a valid license from the Medical and Dental Council of Nigeria (MDCN).DENTAL HOSPITAL
A Dental Hospital is a healthcare institution where services relating to the care of the teeth, oral cavity, and associated structures are provided on an outpatient and (or) in-patient basis.
MINIMUM REQUIREMENTS
In addition to meeting the standards described below, all dental facilities shall possess a Corporate Affairs Commission (CAC) certificate. The Practitioner-In-Charge shall be a fully registered dental surgeon with requisite experience. (S)he shall have a valid license from the Medical and Dental Council of Nigeria (MDCN).MEDICAL LABORATORY
A MEDICAL LABORATORY is a centre for investigation of medical disorders. All medical laboratories are classified according to the scope of services provided.
a. Primary Medical Laboratory: These laboratories perform simple laboratory tests and can be managed by a registered Medical Laboratory Scientist. They include primary healthcare laboratories, side Laboratories within clinics and Point of Care Testing (POCT).
b. Secondary Medical Laboratory: These laboratories carry out diagnostic services as required by the community and may serve as research centres. A secondary laboratory shall be headed by a Medical Laboratory Scientist.
c. Tertiary Medical Laboratory: These are laboratories that can carry out diagnostic services as well as training and research at a sophisticated level. They can also act as reference laboratories. A tertiary laboratory shall be headed by a Medical Laboratory Scientist.
MINIMUM REQUIREMENTS
In addition to meeting the standards described below, all medical laboratories shall possess a Corporate Affairs Commission (CAC) certificate. The practitioner in charge shall be a fully registered Medical Laboratory Scientist with at least 5 years of post NYSC work experience. He/she shall have a current license from the appropriate regulatory body; The Medical Laboratory Science Council of Nigeria (MLSCN).NURSING AND MATERNITY HOME
A NURSING AND MATERNITY HOME is a health establishment aimed to serve clients requiring Preventive, Therapeutic and Rehabilitative Nursing care and also specializes in caring for women during Pregnancy and child birth, as well as other services.
A Nursing and Maternity Home is expected to have a Registered Nurse/Midwife (RN/RM) with evidence of prompt access to referral to a standard medical facility. They are to provide 24 hours services and are expected to have a registered midwife at every point in time during opening hours.
MINIMUM REQUIREMENTS
In addition to meeting the standards described below, all Nursing and Maternity Homes shall possess:- A Corporate Affairs Commission (CAC) certificate.
- Certificate of professional staff (Certificate of Registration with Regulating Body)
- S(he) shall have a valid practicing license from Nursing and Midwifery Council of Nigeria (NMCN).
- The Practitioner-In-Charge shall be a fully Registered Nurse/Midwife (RN/RM) with minimum of five (5) years’ working experience.
- Three (3) years tax clearance certificate.
- Edo State Waste Management receipt
- A letter of commitment to share patient attendance, morbidity, mortality statistics and other relevant data with Ministry of Health on monthly basis.
ELDERLY/CARE HOMES
An Elderly/care Home is a health facility aimed to serve the elderly requiring care which include:
- General Nursing Care
- Special Nursing Care
- Nutritional Support
- Physiotherapy and Occupational Therapy
- Recreational Therapy
MINIMUM REQUIREMENTS
In addition to meeting the standards described below, all elderly/care Homes shall possess:- A Corporate Affairs Commission (CAC) certificate.
- Certificate of professional staff (Certificate of Registration with Regulating Body)
- S(he) shall have a valid practicing license from Nursing and Midwifery Council of Nigeria (NMCN).
- The Practitioner-In-Charge shall be a fully Registered Nurse (RN) with minimum of seven (7) years’ working experience.
- Three (3) years tax clearance certificate.
- Edo State Waste Management receipt
- A letter of commitment to share patient attendance, morbidity, mortality statistics and other relevant data with Ministry of Health on monthly basis.
CLINIC
A Clinic is a healthcare facility that is primarily focused on the care of outpatients (non-resident patients). Clinics provide medical care including examination, consultation, and treatment on an out-patient basis before referral or transfer to a hospital for further management, if necessary.
MINIMUM REQUIREMENTS
In addition to meeting the standards described below, all Clinics shall possess a Corporate Affairs Commission (CAC) certificate. The Practitioner-In-Charge shall be a fully registered medical practitioner with a minimum of 5 years of post-qualification experience. (S)he shall have a valid license from the Medical and Dental Council of Nigeria (MDCN) and shall be a full-time staff.EMBALMMENT CENTRES, FUNERAL HOMES AND MORTUARIES
A Mortuary is a health service point used for the security and safety of human corpses awaiting identification or removal for autopsy, burial or any other post-death activities (including cremation). It is a health facility which is used for the storage of bodies and may include body viewing area, body preparation room and an autopsy unit used to investigate the cause of death.
CATEGORIES:
Mortuaries are categorized based on body-holding capacity. When attached to a hospital, the ratio of 1 to 10 or 25 beds is applied to determine the minimum body holding capacity.
1. Funeral Homes
- Do not have a cold storage system.
- Can stand alone or be attached to a hospital.
- Have a body holding capacity of 4 maximum
- Can store bodies for a maximum of 96 hours.
- Can perform home embalming services.
- Operated by a certified mortician or an anatomist
- May have a cold storage system.
- Can stand alone or attached to a hospital
- Have a body holding capacity of 20 maximum.
- Can store bodies for up to 4 weeks.
- Can perform home embalming services
- Operated by a certified mortician or anatomist or a pathologist
- Must have a cold storage system
- Can stand alone or attached to a hospital
- Body holding capacity of 20 minimum.
- Can store bodies for longer periods but not more than 6 months
- Must have a well-equipped autopsy room
- Can perform home embalming services
- Operated by an anatomist or an anatomical pathologist
MINIMUM REQUIREMENTS
In addition to meeting the standards described below, all embalmment centres/funeral homes/mortuaries shall possess a Corporate Affairs Commission (CAC) certificate. The Practitioner-In-Charge shall be;- A Certified Mortician with no less than 10 years of experience in “Funeral Homes and Embalmment Centres”.
- An Anatomist from a recognised institution for “Stand-Alone Mortuaries”.
- A Consultant Anatomic Pathologist or a senior registrar in Anatomic Pathology with requisite experience (Fellowship for Consultant, minimum of two years for a Senior Registrar. They shall be fully registered with the Medical and Dental Council of Nigeria (MDCN) with an additional fellowship from the National Postgraduate Medical College of Nigeria or its equivalent registrable with the MDCN. The Senior Registrar shall have passed at the membership of the National Postgraduate Medical College of Nigeria or its equivalent registrable with the MDCN.) for “Hospital Attached Mortuaries”.
PHARMACEUTICAL PREMISES
The Pharmacy Council of Nigeria (PCN) is the Federal Government parastatal established by with ‘’PCN Establishment Act 2022’’. It is charged with the responsibility among others of regulating and controlling pharmacy education, training and practice in all ramification
PROCEDURES FOR REGISTRATION
LOCATION APPROVAL
- Receive request for location approval from the proposed pharmacy along copies of the Cooperate Affairs Commission certificate of business name with a pharmacist as one of the directors(Mandatory)
- The prescribed location inspection fee is paid.
- Core PIC determine inspectors to carry out the location inspection
- Notify the superintendent pharmacist (SP) of visit
- Introduce the inspection team (PCN officer and inspectors)
- Verify the distance of premises in radius to other existing pharmacies according to PCN guidelines
- Measure size/dimensions of the premises in line with the practice category where applicable
- Check that the siting and structure are adequate
- Note areas for compliance
- Discuss findings at post Pharmaceutical Inspection Committee (PIC) meetings
- Communication the PCN
- Receive request for pre-registration inspection from the superintendent pharmacist of the proposed pharmacy accompany with copies of letter of appointment of the SP, acceptance letter by the SP and current licence of practice of the SP.
- The prescribed fee for pre-registration is paid.
- Notify the superintendent pharmacist of the visit
- Request for the superintendent pharmacist's current annual license and the names and addresses of the company directors
- Confirm practice category
- Check neatness of premises and surroundings, confirm layout within the pharmacy is professional with pharmacist office, store, counselling area, dispensary and reception are well demarcated
- Check demarcated areas for OTC and ethical and poison cupboard
- Verify adequacy of storage area and confirm presence of safety measures
- Confirm current recommended official books
- Fill inspection check list
- Present report at Post PIC meetings
- Inform the registrar of the recommendation of the PIC
- Inform the SP of the outcome and any compliance issues observed.
PATENT AND PROPRIETARY MEDICINE VENDORS
The licensing authority is the Pharmacy Council of Nigeria established by ’’ PCN Establishment Act of 2022’’.
- ELIGIBILTY: An applicant shall produce evidence to the licensing authority ;
- That the applicant has attained the age of twenty- one(21) years and can read and write.
- Certification of good character by referees
- For pharmacy technician, must have obtained the Pharmacy Technician certificate from PCN accredited institution.
- REQUIREMENTS FOR AN APPLICATION:
- An application in applicants own handwriting indicating the exact location and address where the intended business is to be undertaken and addressed to the Registrar, Pharmacy Council of Nigeria (PCN) and submitted at the PCN State office at the Federal Secretariat , Aduwawa, Benin City.
- Attached copies of Pharmacy Technician certificate and current annual permit.
- Letter of recommendation from two (2) reputable referees; one of whom shall be a registered and currently licenced pharmacist.
- Three passport photographs, one endorsed by one of the referees
- Each application is accompanied with a non- refundable fee as specified by the PCN.
- PROCEDURES FOR THE ISSUANCE OF THE LICENCE
- Submission of a duly completed application form
- Interview of the applicant by the State Patent and Proprietary Medicine Vendors Licence Committee.
- Payment of the prescribed fee
- Inspection of the proposed shop by the State PPMVL committee
- A satisfactory report to the PCN by the State PPMVL committee
- Issuance of the license by the PCN on payment of the prescribed registration fee
- The applicant is issued a booklet containing the list of medicines approved for sale by the licencing authority (PCN)
- The applicant mandatory attendance of a Patent and Proprietary Medicine Vendors (PPMV) orientation course.
